asya śrīśaktipañcākṣaramahāmantrasya vāmadeva ṛṣiḥ। anuṣṭup chandaḥ। śrī umāmaheśvaro devatā। naṃ bījam। maṃ kīlakam। śrī umāmaheśvaraprasādasiddhyarthe jape viniyogaḥ।

nānālaṃkārayuktaṃ varaparaśumṛgābhītihastan triṇetraṃ vande bā(phā)lendumauliṃ gajavadanaguhā'siliṣṭapārśvaṃ maheśm। pañcapūjā। hrīṃ oṃ namaś'sivāya hrīṃ॥ iti ai(śvar)yapradhāno'yaṃ mantraḥ caturdaśada(la?)kṣaṃ jape siddhiḥ।
