oṃ śrīparadevatāyai namaḥ। caṇḍikāyai namaḥ। gurubhyo namaḥ। oṃ śrīpañcākṣarastotramahāmantrasya vāmadeva ṛṣiḥ। anuṣṭupchandaḥ। sāmbasadāśivo devatā। oṃ bījam। naṃ śaktiḥ। maṃ kīlakam। sāmbaśivasadāśivaprasādasiddhyarthe je[ja]pe viniyogaḥ।
